1. Open the appropriate set of forms.
Standard Forms (Format: Word document):
- Adult Intake Forms (Ages: 18+)
- Minor Child Intake Forms (Age 17 and below)
- Adolescent Self-Report (Ages: 13-17)
2. Fill out electronically and email to your therapist, or print and bring to your first appointment.
